“Anatomy of an Incident and Lessons Learned: Arc Flash Accident at Los Alamos National Laboratories (LANL) Technical Area 53,” an Integrated Safety Management (ISM) colloquium, will be presented by Tommy R. Martinez, deputy chief electrical safety officer at LANL, on Tuesday, Aug. 29, 2017, at 10 a.m. in the Bldg. 402 Auditorium.

Abstract
On May 3, 2015 at LANL Technical Area 53, during Preventive Maintenance on a 13.8-kV electrical substation, a wireman entered a cubicle on the energized portion of the switchgear to clean it with a commercial spray cleaner and suffered severe injuries from the resulting arc flash and blast. This presentation will breakdown the accident, describe the findings from the Joint Accident Investigation Team, and discuss the lessons learned and enhancements made to the LANL Electrical Safety Program.
Biography
Tommy R. Martinez is a technical staff member at Los Alamos National Laboratory. He graduated from New Mexico State University in 2005 with a B.S. in electrical engineering. He started his career as a facility design electrical engineer at the Los Alamos Neutron Science Center, with more than 10 years of experience in designing and maintaining a laboratory facility. Martinez has more than 13 years of experience in the field of electrical safety and has served as the division electrical safety officer for one of the major programmatic divisions at LANL, the Accelerator Operations Technology Division, and was the Facility Division electrical safety officer for the Los Alamos Neutron Science Center. He is currently the deputy chief electrical safety officer at LANL. Martinez has served on LANL’s top level of Electrical Safety Authority Having Jurisdiction, the Electrical Safety Committee, and has served as a leader in the development and implementation of the Electrical Safety Program at LANL. He is a leader in the implementation of risk assessment in electrical safety programs at LANL and across the U.S. Department of Energy complex.
